ONLINE BENEFITS ENROLLMENT
With the PlanSource Online Enrollment System, you and your family can access your benefits information
whenever you want, from home or any place where you have internet access. Use PlanSource to view plan
details, coverage amounts and costs.
TO ENROLL OR MAKE CHANGES TO YOUR BENEFITS:
Log in to https://plansource.ultipro.com. You’ll be asked to enter your user name and your
password:
• User Name: Your Puma email address.
• Password: Enter the password that you have elected. You may contact the Human Resources
Department to reset your password, if needed.
It is very important that you login to PlanSource and verify your personal information, elect or decline
benefits, and identify a life insurance beneficiary.
