Cigna voluntary long tErM DiSaBility
United Pacific offers you the opportunity to purchase Voluntary Long Term Disability (LTD) income replacement at
discounted group rates, through Cigna. If you become totally and permanently disabled, benefits begin 180 days
after the start of your illness or injury and will continue as long as you remain disabled up to Social Security Normal
Retirement Age. LTD works with state disability programs, Social Security, and any other group disability coverage,
to provide you with a combined monthly benefit equal to 60% of your pre-disability earnings to a maximum benefit
of $10,000 per month.

Pre-existing Condition Exclusion for Voluntary Long Term Disability Plan:
If you were diagnosed, consulted or treated for any medical condition 3 months prior to your enrollment in this plan,
the disability caused by this condition will not be covered until you have been on the plan for 12 months. After 12
months of consecutive enrollment, the pre-existing condition will no longer apply.
