Health and Wellness Program
Participate in the Volcom health and wellness program by completing wellness activities like the online health assessment,
online coaching, and telephonic coaching. Log into www.mycigna.com. Look for details on wellness activities throughout
the year.

Smoking Cessation Program
If you want to quit smoking, Cigna’s Quit Today program can help you develop a personal plan to become and remain
®
tobacco-free. Choose from two convenient options – an 8 week telephone program featuring a dedicated wellness coach
or an online for a self-paced program with 8-12 weeks of nicotine replacement therapy (upon speaking with a coach), a
workbook and a relaxation cd – or you may use both options. Call (855) 246-1873 for more information.

IDENTITY THEFT

Volcom provides you with the opportunity to purchase identify theft through LifeLock at discounted group rates. Identity
theft can occur if someone uses your personal information without your permission to commit fraud or other crimes such
as opening new accounts, filing fake tax returns, renting or buying property, etc.

LifeLock uses proprietary technology that searches for a wide range of potential threats to your identity. If LifeLock sees
activity with your personal information, they will alert you by text, email or a phone.

If you become a victim of identity theft while a LifeLock member, a dedicated US-based Identity Restoration Specialist will
work with you from start to finish to help fix the issue. If you have money stolen due to identity theft, LifeLock will replace
it, as provided by your plan.

ONLINE BENEFITS ENROLLMENT

With the online benefits portal through ADP, you can access your benefits information whenever it’s convenient, from home
or any place where you have internet access. Use ADP to enter your benefit choices, to address changes, to modify your
personal information and to add/modify your dependent’s information. You can also use ADP throughout the year to download
benefit forms and view benefit statements. Go to ADP to find our Annual Notices Packet, Cigna Summary of Benefits and
Coverage (SBC), carrier documents, information on employee discounts, company policies, payroll information, and more!

• New Users: Click “Register Now” and use registration code: Volcom-1234
• Returning Users: Use your previously created user name and password

Once you are logged in, go to the Myself Header and select Benefits and then Enrollments. From there, follow the wizard
and submit all of your selections at the end.

It is very important that each employee login to ADP and verify their personal information, elect or decline benefits, and
identify a life insurance beneficiary.
