Voluntary Benefits (continued)
The following voluntary benefits are through Unum, but administered by the Farmington Company. Contact information for the Farmington
Company is on page 27 of this brochure.
These Unum voluntary benefits include annual wellness benefits for routine physicals, checkups, mammograms, pap smears, and more. These
benefits are payable even if your health insurance covers preventive services at 100% - an incentive to stay healthy! Deductions for all plans
listed below are taken on a post-tax basis. For more information, contact a Farmington Company representative. Voluntary benefits are
available to all eligible employees.
Group Accident Insurance
When an accident happens, this policy pays you cash to pay for the unexpected medical and everyday expenses that add up quick ly. This
benefit is not just for emergency treatment, hospital stays and medical exams, but for other expenses you may face, such as broken teeth,
concussions, intensive care unit confinement, emergency room visits, lacerations, transportation, and lodging needs. A spouse and children
are also eligible for this coverage. This coverage also pays benefits for a health screening.
Group Hospital Indemnity Insurance
No matter how good your medical insurance is, when you are hospitalized for an injury or illness there will probably be medic al expenses and
out-of-pocket costs that are not covered. A hospital indemnity insurance policy provides cash benefits to use as you see fit. The benefits are
pre-determined and paid regardless of any other insurance you have. Some advantages to this plan are covered childbirths and a lump sum
payout upon admission to a hospital. A spouse and child(ren) are also eligible for this coverage.
Group Critical Illness
A serious health event such as cancer, heart attack, end-stage renal failure or stroke is not only a life-altering physical event, but a devastating
financial one as well. Group critical illness insurance offers an extra layer of protection in the event of a covered critical illness diagnosis. This
plan provides cash benefits as you concentrate on your recovery. Employee and spouse coverage is available, plus dependent children are
covered at 25% of the employee’s amount at no additional cost. It pays a lump-sum benefit for the first occurrence/initial diagnosis of each
covered critical illness. The coverage also pays benefits for a health screening.
Permanent Life Insurance with Long Term Care Rider
Permanent Life insurance offers protection beyond an individual’s working years, potentially for your lifetime. Some advantages of owning a
Permanent Life policy are a guaranteed death benefit that will never decrease, a guaranteed premium rate that will never increase, cash value
accumulation, and other living benefit options. A spouse and child(ren) are also eligible for this coverage.
With the addition of the Long Term Care Rider, this Permanent Life policy provides financial support in the event an insured requires long
term care through nursing home care, assisted living facility or home health care. Permanent Life goes beyond typical term life insurance.
