         Long Term Disability Insurance


         Reliance Standard | Long Term Disability
         Roll-A-Shade offers you Long Term Disability (LTD) to provide income replacement if you become disabled for an extended period
         of time, through Reliance Standard. If you become totally and permanently disabled, benefits begin 180 days after the start of your
         illness or injury and will continue as long as you remain disabled up to retirement age. LTD works with state disability programs,
         Social Security, and any other group disability coverage, to provide you with a combined monthly benefit equal to 60% of your pre‐
         disability earnings up to a maximum benefit of $6,000 per month.



         Employee Assistance Plan


         Reliance Standard | Employee Assistance Plan
         Employee Assistance Plan (EAP): This plan provides 24/7 phone counseling services, and up to 3 face-to-face visits for you and
         anyone in your household.  The EAP counselors provide counseling for stress, financial stress, work/life matters, and more.

         Online Benefits Enrollment





         With the Ease Online Enrollment System, you and your family can access your benefits information whenever you want, from
         home or any place where you have internet access. Use Ease to view plan details, coverage amounts and costs.

                        •   Username & Password: You will receive an email with information that is unique to you. Please follow the
                           instructions provided

                        Once you are logged into the website, follow the prompts on each page to complete your benefit enrollment.
                        You will be asked to verify that your personal information is correct and enter in any of your dependent
                        information.
