BENEFITS





         Life and AD&D Insurance


         Guardian | Basic Life and AD&D Insurance
         Eligible Employees (contact HR to determine if you are eligible)
         Life insurance protects your family or other beneficiaries in the event of your death while you are still actively employed with the
         company. JONS Marketplace pays for coverage, offered through Guardian for eligible employees. If your death is due to a covered
         accident  or  injury,  your  beneficiary  will  receive  an  additional  amount  through  Accidental  Death  and  Dismemberment  (AD&D)
         coverage.

         Supplemental Benefits


         You may purchase individual policies from Sun Life including Voluntary Life and AD&D, Voluntary Accident, and Voluntary Short
         Term  Disability.  Your  premiums  are  paid  through  payroll  deductions  on  a  after-tax  basis.  Sun  Life  policies  offer  direct-to-the-
         policyholder cash payouts to help cover what other insurance doesn’t. All of the Sun Life individual policies are either portable or
         convertible, which means that you can keep them should you change jobs or retire.   Certain restrictions may apply.

         Sun Life | Voluntary Life and AD&D
         In addition to the company provided Basic Life and AD&D benefits, you may elect to purchase additional Term Life and AD&D
         insurance  at  discounted  group  rates  provided  by  Sun  Life.  If  elected,  you  pay  for  this  coverage  with  after-tax  dollars  through
         convenient payroll deductions. Please note that age reductions apply.

         Available coverage options:
          Employee: You may purchase coverage for yourself in increments of $10,000 up to a maximum benefit of $250,000, not to
            exceed 5 times your annual salary.
          Spouse: If you buy coverage for yourself, you may also purchase coverage for your eligible spouse. Benefits for your spouse are
            available in increments of $5,000 to a maximum benefit of $125,000 and may not exceed 50% of your employee election.
          Child(ren): If you buy coverage for yourself, you may also purchase coverage for your eligible dependent child(ren). Benefits
            for your child(ren) are available in the amount of $1,000, $5,000, or $10,000.

         Guarantee issue is a pre-approved amount of coverage that does not require you to provide proof of good health, and is available
         to  you  during  your  initial  eligibility  period  (upon  hire).  Guarantee  issue  is  available  in  the  amount  of  $180,000  for  employee
         coverage, $50,000  for spouse coverage, and $10,000  for child coverage.

         If you are no longer in your initial eligibility period, you may enroll in Voluntary Life and AD&D insurance anytime during the year as
         long as you provide proof of good health. To provide proof of good health, you will be asked to complete a health questionnaire
         and are subject to insurance carrier approval. Sun Life may approve or decline coverage based on a review of your health history.

         Sun Life | Voluntary Accident

         Sun Life Accident insurance will pay you a benefit to help cover your out-of-pocket medical costs in case of an accidental injury. For
         covered accidents, fixed benefits are paid directly to you, regardless of any other coverage. Benefits are paid according to a fixed
         schedule that includes benefits for emergency room visits, hospitalizations, burns, skin grafts, coma, concussion, and dislocations
         (with reduction under general anesthesia), emergency dental work, eye injuries, fractures, lacerations, paralysis, and more.  An
         accidental death and dismemberment benefit is also provided.

         Sun Life | Voluntary Short Term Disability
         JONS Marketplace offers you the opportunity to purchase Voluntary Short Term Disability (STD) income replacement at discounted
         group  rates,  through  Sun  Life.  This  plan  provides  a  high  level  of  income  replacement  for  the  first  24  weeks  of  a  disability.  In
         California, STD integrates with state disability, which means that when you are out on disability, the state will pay first. For 2018
         the weekly benefit is 60% to 70% (depending on your income) of average weekly earnings up to a maximum weekly benefit of
         $1,216.
