ENROLLMENT INFORMATION
Who May Enroll
If you are a regular full‐ me employee working at least 30 hours per week, you and your eligible dependents may par cipate in
Rampart Supply’s benefits program. Your eligible dependents include:
Legally married or common‐law spouse
Same‐sex domes c partner
Children under the age of 26, regardless of student or marital status
When You Can Enroll
As an eligible employee, you may enroll at the following mes:
As a new hire, you may par cipate in the following benefits:
Medical/HRA, Dental & Vision: Eligible to enroll on the first day of the month following 30 days of full‐ me employment
Life/Disability: Eligible to enroll on the first day of the month following 1 year of full‐ me employment
Long Term Care: Eligible to enroll if employed at least 6 months prior to open enrollment
Legal Shield & Iden ty The : Eligible to enroll on the first day of the month following 30 days of full‐ me employment and
can elect any me throughout the year
Each year, during open enrollment
Within 30 days of a life event as defined by the IRS (see Changes To Enrollment below)
Paying For Your Coverage
The Basic Life/AD&D, Long Term Disability, Short Term Disability, and base Long Term Care benefits are provided at no cost to you
and are paid en rely by Rampart Supply. You and the company share in the cost of the Medical benefits you elect. Any Dental,
Vision, and Supplemental benefits you elect will be paid by you at discounted group rates. Your Medical, Dental, and Vision
contribu ons are deducted before taxes. Paying for benefits before‐tax means that your share of the costs are deducted before
taxes are determined, resul ng in more take‐home pay for you. As a result, the IRS requires that your elec ons remain in effect for
the en re year. You cannot drop or change coverage unless you experience a life event.
Changes To Enrollment
Our benefit plans are effec ve January 1st through December 31st of each year. There is an annual open enrollment period each
year, during which you can make new benefit elec ons for the following January 1st effec ve date. Once you make your benefit
elec ons, you cannot change them during the year unless you experience a life event as defined by the IRS.
Examples include, but are not limited to the following: Change in your residence or workplace (if your benefit
Marriage, divorce, legal separa on or annulment op ons change)
Birth or adop on of a child Loss of coverage through Medicaid or Children’s Health
A qualified medical child support order Insurance Program (CHIP)
Death of a spouse or child Becoming eligible for a state’s premium assistance
A change in your dependent’s eligibility status program under Medicaid or CHIP
Loss of coverage from another health plan A spouse or dependent’s open enrollment
Coverage for a new dependent is not automa c. If you experience a qualifying event, you have 30 days to update your coverage.
Please contact the Human Resources Department immediately following a qualifying event to complete the appropriate elec on
forms as needed. If you do not update your coverage within 30 days from the qualifying event, you must wait un l the next annual
open enrollment period to update your coverage.

locate network providers, manage your claims, obtain health and wellness informa on, and
