Life Insurance and Disability Insurance Benefits

Reliance Standard | Basic Life and AD&D Insurance
360 Behavioral Health offers you Basic life insurance which protects your family or other beneficiaries in the event of your
death while you are still actively employed with the company. 360 Behavioral Health pays for $50,000 in coverage, offered
through Reliance Standard. If your death is due to a covered accident or injury, your beneficiary will receive an additional
amount equal to your life insurance through Accidental Death and Dismemberment (AD&D) coverage. For questions
regarding your specific coverage amount, please contact Human Resources.

Reliance Standard | Voluntary Life and/or AD&D Insurance
Voluntary life and AD&D insurance is available for you, your spouse and child(ren) through Reliance Standard on the first of
the month following 30 days of employment. You must enroll yourself to elect benefits for your dependents. Employees are
responsible for the full cost of voluntary life insurance, and if you leave 360 Behavioral Health, you can take your coverage
with you. See Human Resources for rates.

 Employee Coverage: Choose from $20,000, $50,000, $75,000, $100,000, $125,000, $150,000, $200,00, $300,000,
$400,000, or $500,000
 Spouse Coverage: If you elect coverage for yourself, you may purchase coverage for your spouse in increments of $5,000
up to a maximum benefit of $250,000
 Eligible Dependent Child(ren) Coverage: 14 days old through age 19: Choose from $1,000 increments to $10,000. Choose
one benefit amount for all eligible children in family

During this Open Enrollment only, you will be able to elect up to the following guaranteed issue amounts without having to
answer any medical questions. Any amount elected over the guaranteed issue amounts below will require an Evidence of
Insurability form that is located on the EaseCentral portal.
• Employee: $50,000
• Spouse: $10,000
