Benefits

Disability Insurance

If you are unable to work due to an illness or injury, our disability plans will work together to provide a source of income to meet
your needs. Disability insurance provides benefits that replace part of your lost income when you become unable to work due to a
covered injury or illness.

California State Disability (SDI)
If you experience a Short Term Disability claim, you are eligible for benefits provided by the state. California State Disability
Insurance (SDI) is a partial wage-replacement insurance plan for California workers. The SDI program is state-mandated and funded
through employee payroll deductions. SDI provides short term benefits to eligible workers who suffer a loss of wages when they
are unable to work due to a non-work-related illness or injury, pregnancy, or childbirth. For more information on eligibility, benefit
amounts, and instructions on how to file a claim for the SDI program please visit the EDD website at www.edd.ca.gov.

UNUM | Voluntary Short Term Disability Insurance
California Employees Only

Evisions offers you the opportunity to purchase Voluntary Short Term Disability (STD) income replacement at discounted group
rates, through UNUM. If you experience a temporary disability, benefits begin 7 days after the start of your accident, sickness or
pregnancy and will continue up to a maximum of 12 weeks. California State Disability provides a benefit of 60%-70% of your pre-
disability earnings up to a maximum benefit set by the state of California. This Voluntary STD plan pays in addition to California
State Disability an additional 25% of your pre‐disability earnings up to a maximum benefit of $2,500 per week.

UNUM | Short Term Disability Insurance
Non-California Employees Only
This coverage is provided by Evisions to employees residing outside of California at no cost to you.

Evisions offers you Short Term Disability (STD) to provide income replacement if you become disabled due to an accident, sickness
or pregnancy, through UNUM. If you experience a temporary disability, benefits begin on the 8th day after the start of your
accident, sickness or pregnancy and will continue as long as you remain disabled up to a maximum of 12 weeks. STD works with
state disability programs, Social Security, and any other group disability coverage, to provide you with a combined monthly benefit
equal to 60% of your pre‐disability earnings up to a maximum benefit of $2,500 per week.

UNUM | Voluntary Long Term Disability Insurance
California & Non-California Employees
Evisions offers you the opportunity to purchase Voluntary Long Term Disability (LTD) income replacement at discounted group
rates, through UNUM. If you become totally and permanently disabled, benefits begin 90 days after the start of your illness or
injury and will continue as long as you remain disabled up to age 65 or 24 months of coverage, depending on the age when you
became disabled. LTD works with state disability programs, Social Security, and any other group disability coverage, to provide you
with a combined monthly benefit equal to 60% of your pre‐disability earnings to a maximum benefit of $10,000 per month.
