Supplemental Benefits

You may purchase additional insurance from Unum that will help to cover additional out-of-pocket expenses. These policies offer
direct-to-the-policyholder cash payouts to help cover what other insurance doesn’t. Your premiums are paid through payroll
deductions on an after-tax basis. These Unum policies are portable, which means that you can keep them should you change jobs
or retire, with no increase in premiums.

UNUM | Accident Insurance
Unum Accident insurance will pay you a benefit to help cover your out-of-pocket medical costs in case of an accident. For covered
accidental injuries, fixed benefits are paid directly to you, regardless of any other coverage. Benefits are paid according to a fixed
schedule that includes benefits for hospitalization, fractures and dislocations, emergency room visits, major diagnostic exams,
physical therapy, and more. This plan will pay benefits for an off-the-job or on-the-job accident. This plan is guarantee issue and
does not have any pre-existing condition limitations!

Pet Discount Program

United Pet Care (UPC) | Discount Plan

With the United Pet Care (UPC) discount plan, you can reduce the cost of your veterinary bills by saving up to 50% on each
veterinary visit for your pet when you visit a contracted UPC provider. This plan offers discounts for office visits, procedures,
medications, and more. With UPC’s program, there are no deductibles, no claim forms, no exclusions due to pre-existing conditions
or age, no annual dollar limitations, no waiting periods, and no limit on the number of visits annually. Two discount program
