With the Maxwell Health Online Enrollment System, you and your family can access your benefits information whenever you want,
from home or any place where you have internet access. Use Maxwell Health to view plan details, coverage amounts and costs.
The website will walk you through each step of the process, from start to finish.

1. You will receive a welcome email from Evisions prompting you to log into Maxwell.
• If you are a first-time user, you will be prompted to create a password. Your user name will be your email.
• If you are already registered, log in to your account at www.maxwellhealth.com.

2. Confirm your choices:
• Once you review your options and make your elections, you will be prompted to add your dependent and beneficiary
information.
• Ensure your personal information, dependent information, and plan selections are correct.

After confirming your selections, you will have one final chance to review your enrollment selections. Double-check that all of your
information is up-to-date. If you are enrolling any dependents, be sure that each dependent is selected under the applicable
benefit plan. If you need to make any changes, simply do so through our admin site at www.maxwellhealth.com.

Print the confirmation page and keep it for your records.

Waive Coverage
If you choose to waive medical, dental, or vision coverage, you must still log in to the www.maxwellhealth.com and complete the
benefits enrollment process. You will be given the option to select “Waive” coverage.

Please note that, as you are automatically provided with Basic Life Insurance through Evisions, Inc., you will need to select a
beneficiary and enter their information.
