MEDICAL INSURANCE
UnitedHealthcare | High Deductible Health Plan (HDHP)
With the High Deductible Health Plan (HDHP) through United Healthcare, you can pay for qualified healthcare expenses now and
grow your savings for future healthcare needs. With the HDHP, you are responsible for all medical and pharmacy out of pocket
expenses, up to the deductible. Once you meet your deductible requirements, the plan coinsurance and copays will become
activated. Similar to the PPO plan, you have the freedom to choose your doctor without the requirement of selecting a PCP and
you may self-refer to specialists. You may use a UnitedHealthcare ChoicePlus provider whose negotiated rates provide richer levels
of benefits with claim forms filed by the providers. You may also obtain services using a non-network provider; however, you will
be responsible for the difference between the covered amount and the actual charges and you may be responsible for filing claims.
Unlike a traditional medical plan, the HDHP has the option to combine a special, tax-qualified Health Savings Account (HSA) to the
medical plan. You can contribute tax-free money to your HSA, up to IRS maximums. The money in your account is yours to pay for
current healthcare expenses for you and your tax dependents - or you can save toward future healthcare expenses.

UnitedHealthcare | PPO Medical Plan
The UnitedHealthcare Preferred Provider Organization (PPO) plan allows you to direct your own care. The plan has a calendar year
deductible for most services. If you use a participating network provider, you pay just the office co-pay for the visit.
The deductible does not apply to prescription drugs. You will be responsible for the appropriate plan copay for any prescription
medication.
If you receive care from a physician who is a member of the UHC Choice Plus network, a greater percentage of the entire cost will
be paid by the insurance plan, however, you are not limited to the physicians within the network and you may self-refer to
specialists. If you obtain services using a non-network provider, please note that you will be responsible for the difference between
the covered amount and the actual charges, and you may be responsible for filing claims.

NOTE: UnitedHealthcare Managed Indemnity benefit remains available for employees located in Alaska and Yanbu
OptumRx | Prescription Drugs
For UnitedHealthcare Members Only
If you are enrolled in a UnitedHealthcare medical plan, the pharmacy benefit service is administered by OptumRx, a UnitedHealth
Group Company. You are required to visit a participating pharmacy.
The mail order drug program supplements the drug card plan by providing a low-cost method to obtain prescription medication for
periods up to 90 days. A completed order form, a prescription, and a payment for the copayment is all you need.
BlueCross BlueShield of Alabama | EPO Medical Plan
With the BlueCross Blue Shield of Alabama (BCBS of AL) Exclusive Provider Organization (EPO) plan, you may self-refer to any
physician within the BCBS of AL PPO network. This plan requires that you only utilize doctors, clinics, and hospitals that belong to
the network, except in the case of an emergency, however, you may self-refer to specialists. If you choose to see a provider who is
not part of the network, you will be required to pay the full cost for your care.

HMSA | PPO Medical Plan Hawaii only
In Hawaii, the HMSA PPO plan is offered as an option to Kaiser HMO.
