          To qualify for the discounted non-tobacco rates you must either sign a Non-Tobacco User Affidavit or complete a 5.11
          Tobacco-Cessation Program.



          ONLINE BENEFITS ENROLLMENT





          UltiPro is 5.11’s on-line enrollment system. Everyone must log-in to make their benefit elections and confirm that their
          personal information is accurate.




                          TO LOG IN TO ULTIPRO:
                          •   Go to https://ew13.ultipro.com.
                             •   Enter you user name which is the first letter of your first name, your entire last name, and
                                 the last four digits of your social security number (example: jsmith5555). User name is not
                                 case sensitive.
                             •   Enter your password. If you’ve forgotten your password, please click the Forgot Password
                                 link to change it.
                             •   Click Login.
                          •   Click on Menu in the top left corner, then click on Myself and then Open Enrollment.
                             •   Choose the Open Enrollment 2019 session to complete your benefit elections during our
                                 annual Open Enrollment period.
                             •   Choose Life Events for selections/changes related to new hire enrollments or other qualifying
                                 life events. Your HR department will be able to assist you with any questions.
