ID Cards
Mailed and/or electronic ID Cards can take 7-10 business days from the date the insurance carrier
receives enrollment. Often times, a mailed ID card will come in a white non-disclosed envelope.
Please know that providers can verify eligibility with the policy and your Social Security Number.
• Aetna Medical, EyeMed Vision, FSA/HSA card: ID card will be mailed to your home.
• Dental: Online only. Although a physical ID card is not required, you may register as a member online at
www.deltadentalins.com to download one. User type: Enrollee/Adult Dependent and Social Security Number
(SSN) and date of birth will be required to set up an account. This is a California based plan arranged by Delta
Dental of California.
