When You Can Enroll
Dental Insurance Eligible employees may enroll at the following times:
- Dental Options 6 • During open enrollment.
• As a new hire, you may participate in the company’s Medical,
Dental, Vision, and Flexible Spending Accounts on the first day of
Vision Insurance 6 the month following the completion of 30 days of full-time
employment.
Basic Life and AD&D Insurance 7 • As a new hire, you will automatically be enrolled in the company
-paid Basic Life and AD&D, Employee Assistance Program and
Long Term Disability plans on the first day of the month

following the completion of 30 days of full-time employment.
• You may enroll in the 401(k) on the first of the month following

Paying For Your Coverage
Flexible Spending Accounts
- Health Care Spending Account (HCSA) 8 The Basic Life and AD&D, Long Term Disability, Employee Assistance
- Dependent Care Assistance Plan (DCAP) 8 Program and Flexible Spending Account benefits are provided at no
cost to you and are paid entirely by Shepard Bros. You and the
company share in the cost of the Medical, Dental and Vision benefits
Wellness Initiative Program 9 you elect. Your contributions are deducted before taxes are
withheld, which saves you tax dollars. Paying for benefits before-tax
Monthly Contributions 10 means that your share of the costs are deducted before taxes are
determined, resulting in more take-home pay for you. As a result,
the IRS requires that your elections remain in effect for the entire

year. You cannot drop or change coverage unless you have a status
change. For a list of monthly employee contributions, see page 10.
Opt-Out of Coverage
Should you choose to opt–out of the Medical, Dental and Vision plans, you may be eligible to receive $150 per month for waiving
Medical coverage, $25 per month for waiving Dental coverage or a maximum of $175 per month if you waive both Medical and Den-
tal coverage ($2,100 annually). Should you elect to opt-out, this money would be taxable income through payroll in lieu of participat-
ing in the health insurance program. In order to qualify, you must have other employer sponsored coverage in place through your
spouse’s employer and provide proof of that coverage on an annual basis. Individual coverage is not accepted as alternate coverage.
Employees choosing to opt-out may participate in the Basic Life and AD&D, Long Term Disability and 401(k) plans.
