Basic Life and Accidental Death & Dismemberment
GUARDIAN | BASIC LIFE AND AD&D
This coverage is provided by Ria at no cost to you.
Life insurance protects your family or other beneficiaries in the event of your death while you are still actively employed with the
company.
Accidental Death and Dismemberment (AD&D) coverage provides an additional benefit to your beneficiary if your death is due to a
covered accident or injury. Life benefits will reduce by certain percentages as you age, and will terminate when you leave the company
or retire.
Basic Life and AD&D
1.5 times your annual base salary to a maximum benefit of $250,000
EXCLUSIONS AND LIMITATIONS
You must be working full-time on the effective date of your coverage; otherwise, your coverage becomes effective after you have
completed a specific waiting period. Employees must be legally working in the United States in order to be eligible for coverage.
Underwriting must approve coverage for employees on temporary assignment: (a) exceeding one year; or (b) in an area under travel
warning by the US Department of State. Subject to state specific variations.
• Basic Life and AD&D coverage terms on last day of work.
• Life Insurance can be continued while on FMLA up to the end of the total leave period or 12 weeks, whichever is less, in any 12
month period.
CHOOSING A BENEFICIARY
A beneficiary is a person or entity who you designate to receive your death benefits.
Choosing a beneficiary and keeping your beneficiary up-to-date is an essential part of
owning life insurance. Please remember to review your beneficiary designation as new
situations arise, such as the birth or adoption of a child, marriage, or divorce.
