BENEFITS ENROLLMENT
Effective January 1, 2019
Annual Open Enrollment 2018
DECEMBER
Employee benefits open enrollment will be held from Thursday, November 29th to S M T W T F S
Thursday, December 13th for benefits effective, January 1, 2019. We have lots of
exciting improvements this year, including a new benefits online enrollment platform, 1
Paycom!

Our plan year will begin January 1st through December 31st and this will be your
annual opportunity to make your benefit choices for 2019. During this time, you can
change your plan options and add or remove dependents from your coverage. No 9 10 11 12 13 14 15
changes are allowed after December 13th, or during the plan year, unless you

experience a qualified change in family status as defined by the IRS.
Please refer to the attached Employee Benefits Guide and attend the open enrollment 23 24 25 26 27 28 29
meeting on December 6th at 12:00pm to learn more about the plans. All employees

will be required to go online to enroll or decline at our new Benefits Website:
paycomonline.com. This site will include your ability to enroll, make changes, review
plan summaries and evidence of coverage certificates found in the Document Library. Open Enrollment Meeting
All enrollments and changes made during this time are effective January 1, 2019. Enrollment Due Date
ACTION REQUIRED!

California Eye offers the Paycom Online System for you to process your enrollments. You must log into the site
by December 13th to make your elections effective January 1, 2018.
Login Email: You will receive an email by December 6th regarding your username and password. Once you
receive the email, you will be able to login to Paycom site or app and follow the instructions on page 3.
