Enrollment Information

How to Enroll in Benefits
To enroll in benefits, please complete the following steps:
1. Complete the Wayfinder Election Form. Every eligible employee must complete this form, even if you are declining coverage.
2. Complete the Anthem Blue Cross enrollment form to designate your beneficiary for the employer paid Basic Life and AD&D
coverage.
3. Complete all other necessary forms as required based on your benefit elections.
4. Sign and return the forms to the Human Resources Department before the enrollment deadline.

WayFunds
Wayfinders provides you with monthly WayFunds. You may spend your WayFunds on Medical, Dental and Vision coverage. Each
WayFund is currently equivalent to $1.

The amount of monthly WayFunds you receive is dependent upon your length of employment. When you reach your 3rd or 5th
year anniversary, your WayFunds allotment will increase on the first day of the month following your anniversary date. The table
below illustrates the WayFunds you receive based on your length of employment.

Length of Employment Monthly WayFunds
2 to 36 months 50 WayFunds
37 to 60 months 75 WayFunds

61+ months 100 WayFunds

Please note, unused WayFunds can be added to your pay as taxable income.

After your WayFunds have been applied, any remaining costs will be deducted from your paycheck based on your choice of the
following options:
• Pre-tax dollars from your pay can be used for Medical, Dental, Vision premiums.
• After-tax dollars from your pay can be used to purchase Voluntary Life/AD&D and Long Term Disability insurance.

Take advantage of the online resources available through our insurance carriers. You can
locate network providers, manage your claims, obtain health and wellness information, and
much more!
