With the Anthem Blue Cross Health Maintenance Organization (HMO) plans, you must choose a primary care
physician (PCP) or medical group within the plan’s HMO network. All of your care must be directed through your
PCP or medical group. Any specialty care you need will be coordinated through your PCP and will generally
require a referral or authorization. You will receive benefits only if you use the doctors, clinics and hospitals that
belong to the medical group in which you are enrolled, except in the case of an emergency. Physician’s office
visits are covered at a minimal copay. Other services qualify for the applicable annual deductibles. There are two
Anthem HMO plans available, with the most significant difference being the network of providers, please see the
RESOURCES & CONTACTS section of this guide to find the full network names:
SELECT NETWORK HMO:
This HMO option has a significantly lower per paycheck cost. To access covered care, you must choose a primary
care Physician (PCP) in the Anthem Select HMO Network, which is a narrower network of providers.
FULL NETWORK HMO:
This HMO option is available as a ‘buy-up’ and offers members access to the Anthem Full HMO Network, as well as
has a lower deductible.
