All employees are encouraged to participate in WellBiz’s 401(k) plan. This plan allows you to fund for your retirement with pre‐tax
dollars. You may defer up to 100% of your pay, subject to a $19,500 annual IRS deferral limit in 2020. Eligible participants who are
age 50 or older during the calendar year may make a special “catch-up” contribution to the IRS limit of $6,500 in 2020.

To participate in the 401(k) Plan, you must have attained age 18, and completed one month of service. Once you have met these
requirements, you are then eligible to participate in the plan on the first day of the month following completion of these
requirements. You are 100% vested immediately in all contributions. WellBiz matches 100% on the first 3% of your contribution
with a 50% match on the next 2% you contribute. If you decline coverage initially, you may enroll at a later date.
