Features
• All employees including probationers are eligible
• Requires minimum of 24 hours of hospitalization.
• Employee, Spouse, Dependent Children, Dependent Ineligible Children & Dependent Parents can
be covered
• Choice of Sum Insured – 4,5,6,8,10,12,15 & 20 Lakhs
• Eligible Sum Insured:
• Co. contributes 75% of the (premium+ST) (for self, spouse & dependent children (2), upto the
entitled SI
• One time option for choosing the Higher SI in period of 3 Yrs.
• Changes can be made to only next higher/lower slab
• Changes allowed In case of promotion/marriage, re-marriage/increase-decrease in family size
