Director, Youth Development Supervisor or senior branch person in charge by phone if they are not at the location at the time of accident/incident/situation.
7. Clean Up Area: After given authorization by the Branch Director or FT person-in-charge, have the area of injury cleaned. Use proper procedures in handling bloody or sharp material. Secure all unlocked property of the injured person and hold for the person or the family to pick up later.
8. Complete Accident Report: Be sure to get names of all witnesses, including staff and others. Have staff that were present write a description of the accident and how it was handled. The report and any attached documents are BGCS property and are not to be passed out to the injured party or anyone else unless authorized by the Branch Director or FT person-in-charge.
All incidents that result in the creation of a member injury/illness report must be reported to the parent/guardian either in person or by phone.
9. Follow Up: Boys & Girls Clubs of Greater Scottsdale is concerned about all members. Due to the nature of injury and potential proceeding actions, only the CEO can contact the family to check on the status of the injured member.
10. Digital File: All incidents or injuries, regardless of the severity must be logged into the digital file immediately after the area is secure and all parties are safe.
11. Date of Return: In case of communicable disease, provide parent/guardian with steps necessary for child to return to Club participation. This may include doctor’s note or clearing of infection.
FREQUENCY/TIMING:
Every injury must be documented in digital file.
