Employee Injury or Illness Report

Policy 225: Employee Injury or Illness
Club safety is everyone’s responsibility. If an employee is injured or
 becomes ill, they must be treated immediately. It is important that care is
 taken to keep the employee safe, as well as, refrain from disrupting Club
 Operations in an effort to attend to the matter.

  If an employee injury or illness occurs, immediately and appropriately care for the employee and give whatever first aid is available and necessary. If the Employee has incurred a major injury or illness, like unconsciousness or excessive vomiting, take the employee to get medical attention or call 911. Notify Human Resources as well as the Youth Development Supervisor, Branch Director, or other senior staff if they are not at the location at the time of incident.
If the employee is removed from the premises for emergency services, secure their belongings until retrieved by an authorized person.
Once the employee has been tended to, ensure the area is clear of debris, blood, liquids, or other damaged property. Keep the area clear of staff, members, and/or volunteers. Block off until addressed by maintenance or other Club staff. Use proper procedures when cleaning bodily fluids, sharp objects, etc. Club Members/Youth are NOT allowed to assist in the clean-up efforts. If direct exposure to bodily fluids occurs during the clean-up process, the exposed employee must document using the Report of Significant Work Exposure to Bodily Fluids Form.
A Workers Report of Injury Form must be completed immediately by the employee. This form is needed even when emergency personnel are not called. The completed form must be submitted to Human Resources within 24 hours of the incident. If the employee is unable to complete the form, the supervisor on duty must complete the form and sign the document as the person completing the form.
All witnesses must complete an incident report that describes in detail, any information they have regarding the incident. The report must include names, location in facility, time, and any other pertinent information regarding the situation. The incident report must be forwarded to Human Resources within 24 hours of the situation.
The forms are property of the Clubs and should not be shared with any other parties. Request for copies must be sent directly to Human Resources.
