4. Clean Up Area: After given authorization by the Branch Director or FT person-in-charge, have the area of injury cleaned. Use proper procedures in handling bloody or sharp material. Secure all unlocked property of the injured person and hold for the person or the family to pick up later.
5. Complete Accident Report: Be sure to get names of all witnesses, including staff and others. Have staff that were present write a description of the accident and how it was handled. The report and any attached documents are BGCS property and are not to be passed out to the injured party or anyone else unless authorized by the Branch Director or FT person-in-charge.
All incidents resulting in a member injury/illness report must be reported to the parent/guardian either in person or by phone.
6. Follow Up: Boys & Girls Clubs of Greater Scottsdale is concerned about all members. Due to the nature of injury and potential proceeding actions, only a member of the Critical Incident Reporting Team may contact the family to check on the status of the injured member.
7. All injuries or incidents, no matter the severity must be logged in the digital file immediately at www.bgcs.org/hub
In the case of an accident that results in a member accident, insurance claim the accident report and the secondary insurance form should be sent together ASAP to the Administrative Office.
Member Accident Insurance Claim: If the member injury or illness will require outside emergency or medical attention, the primary Proof of Loss Form needs to be thoroughly completed.
HIPAA law states that we as an organization, and any individuals affiliated with the organization are not allowed to speak with any doctor, hospital, dentist or insurance company regarding any member’s accident or claim.
Policy No. 121 – Communicable Disease and Head Lice Specific Information
The health and safety of members of Boys & Girls Clubs of Greater Scottsdale is impacted by the Club’s efforts to minimize the risk of spreading communicable disease and educating families on health-related issues. Staff may refer to the AZ Department of Public Health Communicable Disease Resource Guide for information about specific forms of illness.

Communicable Disease
Certain symptoms in children may suggest the presence of a communicable disease. Excluding an ill child may decrease the spread of the disease to others in the Club. Recommended exclusion varies by the disease or infectious agent. Children with the symptoms listed below will be excluded from the Club until symptoms improve or a healthcare provider has determined that the child can return.
