immediately. You cannot agree to “not tell anyone” or “keep it a secret;” you have an obligation to report. The Club and you can be held personally liable for failure to do so. If you have any questions regarding whether what you have heard constitutes harassment, sexual harassment, intimidation, abuse or threats then contact the Director of Human Resources or the CEO for clarification.
Policy No. 224 – Employee Vehicle Accident
Any time an employee is involved in a vehicle accident while on duty, a drug screening is required within 24 hours. The employee must immediately inform their supervisor and Human Resources so that a drug test can be scheduled, and HR Director will notify the CEO and Coo that an accident has occurred.
Driver eligibility is automatically and immediately suspended until cleared by Human Resources and Senior Management. Employee refusal to report for drug screening is grounds for immediate termination. (Note: Specific results of the drug test cannot be shared with CEO, COO - only that an employee passed or failed the drug test. HR will communicate this to senior management to make determination on if/when to release driving suspension.) Also, any Boys & Girls Clubs of Greater Scottsdale staff members who are certified drivers must report all tickets to HR regardless if received on duty or during personal off-duty time.
If the accident occurs after hours or on a weekend:
Branch Director provides employee with ‘Forensic Drug Testing Custody and Control Form’ provided by Quest Diagnostics to each Branch. Employee must be notified to collect both the Employer and Donor copies after completed by the Lab. Employer copy is to be submitted to Human Resources on next business day. Donor copy is for the employee’s record. When this form is utilized, the screening will be charged to BGCS account and no payment will be required at the time of service.
Branch Director must locate an approved drug testing facility, either call Quest Diagnostics at 800-877- 7484 or go to www.questdiagnostics.com and follow these directions to find the nearest location:
• Hover mouse over link that says, “For Companies & Organizations”
• Click on “Employers” on the left side of men
• On the left side you will see a menu of action items, click on “Locate a Collection Site”
• Under section #1, enter zip code only
• Under section #2, select Drug Screen
• Click “Find”
A list of locations will come up and you can view the address, phone number and hours of each location by hovering your mouse over the name of the lab.
Once location has been selected, provide employee with address/directions and have them immediately report to the lab for screening.
Policy No. 225 – Employee Injury or Illness
If an employee injury or illness occurs, immediately and appropriately care for the employee and give whatever first aid is available and necessary. If the Employee has incurred a major injury or illness, like
