SOAFEETM dental plans include The Ameritas Dental Network. Providers have agreed to charge 25-50% less than their regular rates which can lower your out-of-pocket costs. You can visit any dentist, in or out-of-network, (but in-network have better rates), and family members do not need to see the same provider. SOAFEETM also offers AFLAC Argus Dental - Group Dental w/minimum participation requirements currently available only in Florida.

Voluntary Life For Employees
Employees who purchase at least the minimum amount of coverage at the initial annual enrollment ($10,000) can buy up to the non-medical maximum ($100,000 Employees, $25,000 Spouses) at any future annual enrollment without any medical questions asked.
Participation Requirement: 10 employees minimum or 20% of the eligible employees must participate in the plan.

Vision
The SOAFEETM vision plan includes Ameritas VSP, which offers the nation's largest network of independent providers. There are preferred providers where you may deeper discounts. SOAFEETM also offers AFLAC Argus Vision-Group with Minimum partici- pation requirements currently available only in Florida.
