               Dear Ms. Prack:

               Your Medicare evaluation is enclosed. Please check the information on the client data sheet on
               page 5 to make sure it is correct.

               After you’ve reviewed the evaluation and decided on the Advantage plan that you want, you
               can call the plan’s toll-free number, shown on pages 2-3 of your evaluation, to enroll. The three
               Advantage plans in your evaluation all include Part D coverage, and when you enroll in one of
               these plans you will also be enrolling in Part D.

               Make sure that you enroll in Part B within designated time frames

               You are probably already aware of this, but in case you aren’t I’ll mention it. If you try to enroll
               in Part B too late, you might have to wait to enroll and could be subject to late-enrollment
               penalties. Your questionnaire indicates that your Part A coverage began on March 1, and if your
               initial Medicare enrollment period also began on that date, then you may need to enroll in Part
               B fairly soon (by the end of June at the latest).

               You can delay your Part B enrollment, however, if you are currently working for a company with
               20 or more employees and you are getting your health insurance through that company’s group
               plan. Or, if you are covered by your currently working spouse’s group plan, you may be able to
               delay Part B enrollment.

               But you cannot delay Part B enrollment if you are not currently working (or if you do not get
               health coverage through your currently working spouse). Some people who have COBRA
               coverage mistakenly think that they do not need to enroll in Part B until their COBRA coverage
               ends, but because COBRA coverage is for people who are not currently working, it cannot be
               used to delay Part B enrollment.

               The Part B enrollment rules are sometimes complex, and it’s important to be sure you are not
               risking a late-enrollment penalty by waiting until September 1 to begin your Part B coverage.
               Appendix A1 explains the basics of enrollment for people younger than 65, but the best
