that once you’ve paid the Part B deductible, you will not have any co-payments for
                       Medicare-covered services. Today a 65-year-old woman in Franklin County, Ohio, can
                       get Plan G for approximately $1,700 a year (or about $140 a month).

                       Besides Plan G, there are other good Medigap plans that have slightly less
                       comprehensive benefits (and lower premiums). Plans L and N are also solid choices,
                       although in these plans you will have more cost-sharing than you would with Plan G.

               If you decide to get a Medigap policy in the future, you can get an updated list of each
               company’s premiums by contacting the Ohio Senior Health Insurance Information Program
               (OSHIIP) at (800) 686-1526. OSHIIP is a division of the Ohio Department of Insurance. Current
               Medigap premiums are listed in Appendix C.

               Medicare Advantage plans

               Advantage plans are managed-care plans – primarily HMO’s and PPO’s. Most Advantage plans,
               including the three in your evaluation, include Part D (or prescription drug) coverage.

               Whereas Medigap policies have high premiums, Advantage plans often have low or even no
               premiums. That’s the case with the three Advantage plans in your evaluation. If you do not use
               a lot of medical services or have expensive treatments, in most cases you will save money in an
               Advantage plan because you will pay substantially less in premiums than you would if you had a
               Medigap policy.

               Both your doctors are listed in the networks of the three Advantage plans in your evaluation
               (see Appendices B2 and B3) -- but occasionally these listings are out of date. So, before
               enrolling it’s important to verify with your doctors’ offices that they will accept the specific
               Advantage plan that you are going to enroll in.

               Here are summaries of the three Advantage plans compared in your evaluation, all of which
               have above-average four-star quality ratings from Medicare. Detailed benefit summaries of
               each of these plans can be found in the appendices, and if there’s a particular medical service
               that you know you will need during the year, you may be able to see how much it will cost.

                       1)  Humana Gold Plus HMO Plan. As you probably know, in HMO plans you must use
                          the plan’s network providers unless it’s an emergency. With more than 12,000
                          providers in the Columbus Metro area, this plan has a wide choice of doctors if you
                          need to see a new specialist. It also has a very good $4,500 out-of-pocket limit for
                          Medicare-covered services.

                          In addition, this plan has no health plan premiums, no health plan deductible and
                          low costs for the prescription drugs that you take. There are also dental
                          supplements available with plan for an added premium. This plan’s benefit summary
