Voluntary Benefits




Critical Illness

Critical illness insurance can pay a lump sum beneit at the diagnosis

of a covered illness. You can choose the level of coverage and use this
payment any way you like. Examples of covered conditions include heart
attack, blindness, or major organ failure. Some time sensitive examples
include stroke, coma, or paralysis. Pre-existing condition limitations and
guarantee issue amounts apply.


Accident Insurance

For an additional layer of inancial protection, you may choose to
purchase accident insurance. This plan pays beneits for accidents that

occur on or off the job. Examples of covered accidents include broken
bones, eye injuries, cuts repaired by stitches, burns, and some emergency
room treatments. There is no evidence of insurability required for this
beneit.


Hospital Indemnity

The hospital indemnity plan can complement your medical coverage by
paying a cash beneit directly to you whenever you or your covered family
members are admitted to the hospital. Whether you are being treated
on an inpatient or outpatient basis, this coverage will help you manage

your expenses so you can focus more on your recovery and less on your
inances.


Your premiums depend on the level of beneit you choose and must be
paid for on an after-tax basis. Coverage is provided through Unum.
