2. Click on Enroll Now
3. Complete your application and be sure to ill
out the “Medical Information—High Deductible
Health Plan (HDHP)” portion on Step 1—your
group number is 729362
What You’ll Need to Enroll
■ Your Social Security number
■ Your email address
■ The group number from your health plan ID
card (729362)
■ An identiication number from another form of
ID, such as your driver’s license, state-issued
identiication card, or passport
Be on the Lookout
You can choose to receive your welcome kit
electronically or by mail. Within seven to ten days,
you will receive a Health Savings Account Debit
MasterCard® by mail in an unmarked envelope.
