LIFE AND DISABILITY
INSURANCE
Life and disability insurance is moving from Prudential to Cigna.
Basic Life and Accidental Death and
Dismemberment (AD&D)
The company automatically provides you a beneit amount based on if
you are an hourly or salary employee, at no cost to you for life insurance
and accidental death and dismemberment insurance. The basic life beneit
now includes identity theft protection. Additional information will be
provided.
Long Term Disability (LTD)
The company offers long term disability insurance to provide inancial
assistance in the event you are unable to work for an extended period of
time. The plan covers 60% of your pre-disability earnings. The maximum
beneit of base coverage is $6,000 monthly. If you choose to elect this
beneit, you will pay a monthly rate of $0.45 per $100 of your covered
monthly payroll.
