Voluntary Spouse Life

You may also elect voluntary life insurance coverage for your spouse. You
can purchase coverage in increments of $5,000. Your spouse’s coverage
cannot exceed 50% of the employee amount to a maximum of $100,000
and you must elect voluntary life coverage for yourself before you can elect
spousal coverage. The cost per each $1,000 of coverage is the same as the
employee cost. You will need to supply proof of good health if you have

previously waived this coverage, are changing your existing, or electing
coverage greater than the guaranteed issue amount of $30,000. If this is the
case, your elections are subject to approval through our insurance provider,
Lincoln Financial.


Voluntary Dependent Life

In addition, you may elect voluntary life insurance for your dependent
children. You must have voluntary life coverage on yourself before you
can elect dependent coverage. The beneit is a lat amount of $10,000 for
children aged over 6 months ($500 for children aged 14 days to 6 months).

The cost of this coverage is $2.00 per month regardless of the number of
dependents.
