Franke Wellness Program
Franke greatly values wellness, thus we seek to inspire you and your
family’s collective physical, mental, and inancial wellbeing. We offer
signiicant discounts on your medical premiums for completing the
wellness requirements.
As a new hire, you will automatically receive the reduced medical
premium rates. To receive reduced premiums in 2019, all activities must
be completed by October 31, 2018. If you were hired June 1, 2018 or
later, you are not subject to the wellness surcharge for the 2019 plan year.
Participation in the 2018 wellness program is voluntary and all Franke
employees and covered spouses are eligible to participate in the program.
Non-covered spouses and dependents will not be eligible to participate in
the program. From January 1, 2018 to October 31, 2018, employees and
spouses covered under the Franke medical plan will have the option to
complete the 3 wellness requirements outlines below to earn a signiicant
incentive on their 2019 medical premiums.
Requirements
1. Complete the Health Risk Questionnaire (HRQ)

2. Complete the health risk assessment lab with biometrics and a follow-
up provider review
3. Complete two preventive screenings
