LIFE AND DISABILITY


COVERAGE



OCLC partners with The Hartford to provide Life and Disability Insurance at a
variety of coverage levels.


Short Term Disability (STD)
Premiums 100% Paid by OCLC

OCLC provides STD which helps provide financial security until you can return to
work. STD pays 60% of your pre-disability earnings for up to six months.

Long Term Disability (LTD)

Premiums 100% Paid by OCLC

OCLC provides LTD which helps provide financial security when unable to work
for an extended period of time and have exhausted all eligible leave (e.g., sick,
vacation, personal days, and short term disability). LTD pays 60% of your pre-
disability earnings.

Group Term Life (GTL) and AD&D Coverage

Premiums 100% Paid by OCLC

OCLC automatically provides GTL and AD&D coverage. If you are a full-time
employee, you will receive a benefit equal to three times your annual base salary
rounded to the next $1,000 and a benefit equal to $25,000 if you are part-time.

Employee Voluntary Life and AD&D Insurance


You are eligible to purchase voluntary life insurance in increments of 1-5 times
your annual salary up to a maximum benefit of $500,000. The cost is based on
your age and tobacco use status.

Spouse and Dependent Voluntary Life Insurance

You can also elect voluntary life coverage for your spouse/domestic partner
in $10,000 increments up to $250,000. Premiums are based on the age and
tobacco use status of your spouse/domestic partner.

Dependent child life coverage is available in coverage levels of $5,000 ($500
live birth through 14 days old) per child. This plan costs $1.06 per month
regardless of the number of covered dependent children.
