Critical Illness Coverage
Why Critical Illness Coverage?
Efective January 1, 2020 we are partnering with Voya to ofer our
Critical Illness coverage. Critical illnesses can be a major detriment
to your income, health insurance expenses, and daily living. Voya’s
critical illness plan pays a lump sum directly to you for any of the
conditions shown in the chart.
Features
X Guaranteed Issue: No medical questions or tests are required for
coverage
X Flexible: You can use the beneit payments for any purpose you
like
X Portable: If you leave Intertek or retire, you can take your
coverage with you (provision may vary by state)
Covered Conditions and Benefits
You may elect to purchase critical illness coverage up to the limits
shown in the chart.
Covered Person Limits
Employee You may elect a beneit amount of $10,000, $20,000, or $30,000
Spouse You may elect a spouse beneit amount of $5,000,
$10,000, or $15,000
Child(ren) 25% of employee-elected coverage for no additional premium
Percent of Beneit Paid
Heart attack 100%
Stroke 100%
Coronary artery bypass surgery 25%
Cancer 100%
Carcinoma in situ 25%
Benign brain tumor 100% Wellness Benefit
End stage renal (kidney) failure 100%
Major organ failure 100% This beneit pays $75 per calendar
Coma 100% year per insured individual
(employee and spouse) if a covered
Blindness 100% health screening test is performed,
Permanent paralysis 100% including blood tests, chest x-rays,
stress tests, mammograms and
Additional covered conditions for dependent children: 100% colonoscopies. A full list of covered
cerebral palsy, cystic ibrosis, Down Syndrome tests will be provided in your
certiicate.
