Accident Coverage
Why Accident Coverage?
Efective January 1, 2020 we are partnering with Voya to ofer our
Accident coverage. Accident coverage applies to non-work related
accidents only. If you elect to purchase accident coverage, this plan
pays a tax-free beneit directly to you to help ofset unexpected
expenses associated with an accident (of the job only) for yourself or
a covered family member.
Features
X Guaranteed issue: No medical questions or tests are required for
coverage
X Flexible: You can use the beneit payments for any purpose you
like
X Portable: If you leave Intertek (or retire), you can take your
coverage with you
Here is a brief summary of the covered medical treatment or service. A
full list of covered services will be provided in your certiicate.

Accident hospital care
Surgery open abdominal, thoracic $1,200
Hospital admission $1,250
Hospital coninement per day, up to 365 days $375
Transportation per trip, up to three per accident $750
Lodging per day, up to 30 days $180
Accident care Wellness Benefit
Initial doctor visit $90
Urgent care facility treatment $225 This beneit pays $50 per calendar
Emergency room treatment $225 year per insured individual
Ground ambulance $360 (employee, spouse, and children)
Air ambulance $1,500 if a covered health screening
Outpatient surgery (one per accident) $225 test is performed, including blood
X-ray $45 tests, chest x-rays, stress tests,
mammograms, and colonoscopies.
Common injuries A full list of covered tests will be
Burns second degree, at least 36% of the body $1,250 provided in your certiicate.
Emergency dental work $350 crown, $90 extraction
Eye injury surgery $350
Ruptured disk surgical repair $800 Plan Pays You
Concussion $225 Monthly Premium
Dislocations Closed/open reduction
