listed preventive prescriptions subject to a copayment as noted on page

Buy-Up Plan (HDHP)
Offers higher per-paycheck premiums with lower deductibles and out-
of-pocket maximums
A meaningful option for those who prefer to pay more in premiums
throughout the year and less when receiving services after reaching
their deductible
You pay less for non-preventive medical care and prescription drugs
once reaching the deductible
Base Plan (HDHP)
Offers lower per-paycheck premiums with higher deductibles and out-
of-pocket maximums
A viable option for healthy individuals/families who do not anticipate
a great need for care throughout the year, or those who foresee some
large expenses where the out-of-pocket maximum will easily be met
myNEXUS will provide a dollar-for-dollar matching contribution
to your HSA as described further on page 11, up to a plan year
maximum of $600
Like the Buy-Up plan, the Base plan begins paying for non-preventive
medical and prescription drugs once the deductible is met
Find an In-Network Provider
When you choose to visit in-network providers you’ll receive the deepest
level of discount on your services. You’ll also have the most cost
protection from your plan. This is because our in-network providers have
agreed to charge negotiated rates. To ind an in-network provider near
you, visit www.humana.com and search for providers in the Choice Care
PPO or Choice POS networks. If you decide to go to an out-of-network
provider, there are beneits available. However, you will pay more out of
your pocket for care so it is in your best interest to stay in-network.
