Basic and Voluntary Life Full-time employees who First day of the month
Insurance, Dependent work 40 or more hours following 60 days of
Life Insurance and per week employment
Voluntary AD&D
Insurance
Other Voluntary Beneits
Short-Term Disability, Full-time and part-time First day of the month
Long-Term Disability, employees who work 20 following 60 days of
Accident Insurance,
Critical Illness Insurance, or more hours per week employment
Group Legal and Auto &
Home Insurance
Dependent Coverage
In addition to electing coverage for yourself, you can elect to
cover your eligible dependents under your medical (including
prescription), dental, and/or vision beneits. You can also elect
voluntary dependent life, Voluntary AD&D, critical illness, and/or
accident insurance.
Your eligible dependents include your legal spouse and
dependent children up to age 26.
A child is deined as your son, daughter, stepchild, adopted
child, or eligible foster child. Coverage for children can be
elected regardless of student status, marital status, inancial
dependence, or residence.
NOTE: If you experience a Qualifying Life Event you must log in to
www.myAPRBeneits.com and follow the Life Event instructions within 31 calendar
days of the event (unless noted otherwise). Changes that are requested due to
a “change of mind” are not allowed until the next Beneits Enrollment period.
You will also need to provide documentation that supports the life event being
reported.
