* A partial benefit (25%) is payable for carcinoma in situ and coronary artery bypass
surgery . Payment of the partial benefit for carcinoma in situ will reduce the benefit
for cancer . Payment of the partial benefit for coronary artery bypass surgery will
reduce the benefit for a heart attack .
Coverage
X Coverage is available in $5,000 beneit increments up to $20,000 for
an employee and in $2,500 increments up to $10,000 for a spouse; the
spouse amount may not exceed 50% of the employee amount, subject
to the minimum face amount of $2,500
X Each eligible dependent child is covered at 50% of the primary
insured amount at no additional charge
Eligibility
X Issue ages: employee: 18–69, spouse: 18–69, children: under age 26
X All full-time, beneits eligible employees working at least 30 hours
or more per week are eligible; if an employee is eligible, their spouse
and all children of the insured who are less than 26 years of age are
eligible
X If elected, coverage is effective on the irst of the month following
60 days of employment and you must be actively at work on the plan
start date for coverage to be effective
