Your 2019 Benefits Guide


Welcome to 2019 Benefits Enrollment

At Envision AESC US LLC, we offer our employees a competitive and
comprehensive beneits program. This is one of many ways we recognize
how important you are to the company. This beneits guide briely
summarizes our programs in a quick and easy-to-understand way.


New Hire Enrollment

Welcome to our team! As a new employee, you are eligible for coverage
on your irst day of employment.* However, you must enroll in beneits
within 30 days of your date of hire. If you do not elect or decline
coverage within 30 days, you will be automatically enrolled in employee
only coverage for the Savings Medical Plan, the Dental Plan, and the
Base Vision Plan. Additionally, if you do not elect voluntary life coverage
for yourself and/or your spouse when initially eligible and wish to elect
coverage at a future open enrollment, you must respond to medical
questions and be approved by the insurance company in order to be
covered.


* Short term disability coverage is effective on your 30th day of employment; long
term disability coverage is effective after you have been employed for 13 weeks;
Aflac Accident and Critical Illness plans, Hyatt Legal, and InfoArmor coverages, if
elected, are effective on the first of the month following 60 days of employment .
