Important Benefits

Requirements



If You Plan to Opt Out of Envision AESC Medical
Coverage

As an employee, you have the choice to opt out of medical coverage. Envision
AESC offers an opt out credit which is paid each pay period (up to the annual
amounts shown below) if you decline coverage. Please note: if a married
couple works at Envision AESC and both opt out of Envision AESC medical
coverage, Envision AESC will only pay one opt-out credit ($900 Employee +
Spouse coverage or $1,200 Employee + Children/Family Coverage).


If you wish to decline Envision AESC ’s medical coverage and receive the opt
out credit, you must complete your initial enrollment election. If you do not
complete your initial enrollment you, will be enrolled in Employee only Savings
Plan, Dental coverage, and Base Vision coverage.


For instructions on how to complete your enrollment, please follow the
instructions for logging in to the Envision AESC ADP Portal on page 6.

* Your opt out credit is subject to taxes and will be provided each pay period and is pro-
rated based on when your employment begins .


If You Add Dependents to Your Benefits

You are required to provide supporting documentation (marriage certiicate,
birth certiicate, adoption certiicate, etc.) for any dependent you add to your
beneits. This documentation must be submitted to the Envision AESC HR
team as instructed within 30 days after completing your enrollment. Your
dependent’s enrollment will be pended until documentation is submitted
and approved. If you do not provide supporting documentation within
30 days, your dependents will not be covered on your beneits. The types
of documentation needed and how to submit the documents will be
communicated to you by the Envision AESC HR team.
