B Well is available to all associates in 2017. HSA qualiied plan
participants can earn up to $1,000 into their HSA. Those achieving B
Healthy level or higher will not only receive HSA seed money in 2017, but
also a $450 contribution discount off your medical contributions in 2018.

screening!

B Informed 1,000 points $250 HSA seed
B Active 3,000 points $250 HSA seed
B Healthy 5,000 points $250 HSA seed
B Well 7,000 points $250 HSA seed
If you are not eligible to enroll in an HSA (see page 11), you will be
incentivized in another manner. Please contact your HR Beneits team for
more information.
