Basic Life and AD&D Insurance
Emmaus provides a $10,000 benefit at no cost to you through Prudential.
Voluntary Life and AD&D Insurance—Prudential
All active full-time employees are eligible to purchase voluntary life and AD&D insurance
through Prudential. Voluntary Life Insurance can be purchased in increments of $10,000
up to 7 times annual earnings or $500,000. AD&D can be purchased to match your
Voluntary Life coverage amounts.
Please note - Benefits are age-banded and reduce to: 65 percent of your original benefit
at age 65; 50 percent of your original benefit at age 70. *$200,000 is the guaranteed issue
amount. Additional amounts subject to evidence of insurability and approval.
Spouse Voluntary Life—Prudential
Spousal coverage is only available if you elect a minimum
of $20,000 voluntary employee life coverage. Coverage is
available in increments of $5,000 up to 50 percent of your
Voluntary Life amount to a maximum to $250,000, subject
to a $30,000 guarantee issue amount.
Child Voluntary Life—Prudential
Child life coverage is only available if you elect a
minimum of $20,000 voluntary employee life coverage. It
provides $10,000 in coverage from 14 days of age to age 26.
