Medical—Health Alliance Medical Plans (HAMP)
Associates residing in the Champaign/Ford, Macon/Shelby,
McLean/Livingston and Peoria/Tazewell Markets will have HAMP
as their medical administrator and utilize the IL-Employer Group
Network.
Please follow the steps below if you need to locate an in-network provider
or facility.
1. Go to https://www.healthalliance.org/
2. Enter your ZIP code or city and state
3. Select “Find a Doctor or Pharmacy” or once you have your ID card
you can use the login
4. Search by Member ID, Provider Name, or By Network
5. The Network will be “IL-Employer Group”
6. The Directory will be-“Self-Funded/Level Funded”
Health Alliance members will also have access to a nationwide provider
network through MultiPlan-PHCS for providers who are not in the
Health Alliance Employer Group Self-Funded network directory.
Important note: when you receive care in the MultiPlan-PHCS network,
it’s your responsibility to make sure preauthorization is received when
necessary. Please call the Customer Service number on the back of your
ID card for assistance in inding a MultiPlan-PHCS provider and to check
whether preauthorization is needed.
Telemedicine Program
MDLIVE is a telemedicine program offered as a covered beneit in all
medical plans. No appointments, no driving, and no waiting at an urgent
care center. MDLIVE lets you talk face-to-face with a doctor through
your mobile device or a computer with a webcam. MDLIVE can be used
for common health concerns like colds, lu, fevers, rashes, infections, and
allergies. It’s faster, easier, and more convenient than a visit to an urgent
care center. For additional information or to schedule an appointment,
visit www.healthalliance.org/virtualvisits or call 888.912.0904.
