Critical Illness and Accident
TheBANK offers voluntary beneit products through MetLife. These
beneits are available to all full time and part time employees.
Accident
This plan pays a beneit for accidents which occur off the job. Examples
of covered accidents include broken bones, eye injuries, cuts repaired by
stitches, burns, and some emergency room treatments. Your premiums
depend on the tier of coverage you choose.
Accident Plan Bi-Weekly Premium
Employee Only $5 .10
Employee + Spouse $10 .25
Employee + Child(ren) $10 .46
Family $13 .20
Critical Illness
Age Bi-Weekly Premium Critical illness insurance pays a lump sum beneit at the diagnosis of a
covered illness. The beneit amount is $30,000 and is paid depending on

<25 $4 .71 the type of illness diagnosed. This payment may be used any way you
25–29 $4 .98 like. Examples of covered conditions include cancer, heart attack, stroke,
30–34 $7 .06 major organ failure, coma, MS, ALS, and Alzheimer’s. There is no pre-
35–49 $9 .97 existing limitation to the critical illness plan. MetLife pays beneits based
40–44 $15 .51 on date of diagnosis. If the date of diagnosis for any covered condition is
45–49 $23 .54 before the effective date, MetLife will not pay a beneit. All elections are
50–54 $34 .20 guaranteed issue. Your premiums depend on the employee’s age and the
55–59 $48 .05 tier of coverage elected. Employees may elect coverage for themselves,
60–64 $69 .92 their spouses, and their child(ren).
65–59 $105 .65
70+ $162 .28
