Benefits Introduction
It’s time to make some important decisions about beneits for you
and your family. The W.J. Bradley 2015 Employee Beneits package A Message
is summarized in this guide. More speciic plan information is From W.J. Bradley
available in your Summary Plan Descriptions. You may request a
copy of these materials from Human Resources.
All eligible employees may participate in W. J. Bradley’s Health,
Dental, and Vision plans as well as FSA, Basic Life and AD&D,
Voluntary Life and AD&D, Short-Term Disability, and Long-Term
Disability. During annual re-enrollment and new hire enrollment
you have the opportunity to:
X Add, change, or delete coverage
X Add or drop dependents from coverage
X Enroll or re-enroll in the healthcare or dependent care lexible
spending accounts (to continue your FSA beneits, you must
re-enroll each plan year)
Outside of your annual re-enrollment or new hire enrollment
window, you cannot change your beneit elections unless you
experience a qualifying life event.
A qualifying event is a change in your personal life which may
impact your eligibility or a dependent’s eligibility for beneits.
Examples of some qualifying events include:
X Change of legal marital status (e.g., marriage, divorce, death of
spouse, legal separation)
X Change in number of dependents (e.g., birth, adoption, death
of dependent, ineligibility due to age)
X Change in employment or job status
It is your responsibility to notify Human Resources within 31 days
of the qualifying life event. Failure to notify Human Resources
within 31 days will prohibit your ability to change your beneits
election(s).
