Z $2,500 Deductible Plan (70%/30% in-network)
X Employees sign into Workday to elect a health plan,
designate beneiciaries, and/or add dependents. Sign into
Workday by opening the Okta Single Sign-on Portal at

https://wjbradley.okta.com.
X Primary care physician ofice copays differ between primary and
specialty care

X Prescription drug copays are based on the type of medication
prescribed

X Refer to the plan summaries provided in this booklet for additional
information regarding each plan offered

Dental

X Provided on a voluntary basis through MetLife
X Calendar year deductible is $50 for individuals and $150 for family

X Calendar year maximum is $1,500

X Orthodontia is covered at 50% for adults and dependent children
($1,000 lifetime maximum)

Vision
X Provided on a voluntary basis through UnitedHealthcare

X Both the eye exam and materials copay are $10

Life and AD&D

X W. J. Bradley provides $25,000 in Basic Life and Accidental Death
and Dismemberment (AD&D) coverage for each employee

X You may also elect supplemental coverage for yourself and your
family
X Dependents are eligible from 15 days of birth to age 26
