Scheduling
Except in the instance of personal or family illness, all PTO must be
scheduled in advance with your supervisor. Every effort will be made to grant
your request for PTO at the time you desire. However, PTO cannot interfere
with your department’s operation and therefore must be approved by your
supervisor in advance.
In the event that more than one employee requests the same period of time
off, it will be handled at management’s discretion.
Short Term Disability beneits and worker’s compensation beneits may
be coordinated with employee paid time off balances where permitted by
state law. See your supervisor if you have any questions, as criteria for this
coordination varies greatly by state.
When you use your PTO to care for yourself or a family member as Family
Medical Leave the absence will not be included when reviewing your
attendance. Other non-scheduled PTO usage for personal, sick, unscheduled
appointments and personal needs is included and reviewed by your supervisor
to ensure attendance standards are met. This is why it is very important that
you notify your supervisor immediately if you believe that your PTO usage is
for Family Medical Leave usage.
Termination
Employees are encouraged to use their PTO to take regular time off each
year. Employees may carry over a maximum of speciic number of days and
corresponding hours based on the table listed in this section. Exceptions
to this policy may be made in unusual circumstances. Each case will be
considered separately by management. If you have unused and accrued PTO
hours upon the termination of your employment with Americare, with at least
a two week notice, you will be paid a pro-rated portion of your unused time.
For less than 5 years of service will be paid at 50%, 5-10 years of service will
be paid at 67%, 10-15 years of service will be paid at 75% and over 15 years
of service will be paid at 100%.
