• Make appointments
• Turn-by-turn directions and maps to providers
• Eye exam and contact lens reminders
• Electronic ID card for office visits
• Vision prescriptions for everyone on your plan
• Benefit plan details
• Answers to common questions
• Special offers and discounts
• Direct line to EyeMed support
EyeMed participating providers will provide you and
your covered family members may also receive 20%
off any item not covered by the plan, including non-
prescription sunglasses by showing your EyeMed ID card.

the event there is a discrepancy between what is reflected in this
guide and what is communicated in your SPD, the terms of your
