For many people, nothing is more important than our health and the
health of our family members. This is why Murphy Company offers a
robust medical and pharmacy program to employees. Murphy partners
with Anthem to provide these beneits.

As an employee of Murphy Company, you continue to have the option
between three plans—the Premium Plan, Regular Plan, and the HSA Plan.
You are encouraged to weigh the below considerations when choosing the
plan which best meets your needs.

X Highest per-paycheck premiums with lowest deductibles and out-of-
pocket maximums

Important Terms X Flat dollar copays for Tiers 1, 2, and 3 prescription drugs
Deductible—the amount you pay X Flat dollar copays for primary and specialty doctor’s ofice visits
